Permanency and Well-Being

The Department of Permanency and Well-Being currently consists of a director, three Multi -Disciplinary Team (MDT) staff, two Family Finders (FF) Specialists and one Post-Adoption Specialist. The purpose of the MDT staff is to facilitate all staffings of children who are transiting to and from any placements such as foster homes, facilities, relatives, or back home with parents. MDT staffings are held to ensure transition plans will be implemented and children and caregivers are receiving what is needed to ensure a successful move.

Family Finders Specialists are assigned to children who do not have an identified permanency plan or known connections in the community. The Family Finders Specialist works in partnership with the child’s case manager and Guardian ad Litem to ensure the team has input when it comes to connections and placements identified in the search process.

The Post-Adoption Specialist works with families who previously adopted children from the system of care; the specialist’s role is to ensure all post-adoption families receive the support and resources necessary to maintain a successful adoption.
